Postpartum Support Virginia (PSVa) is immediately hiring a Professional Resource and Care Coordinator. PSVa is a nonprofit serving the Commonwealth of Virginia with a two-pronged mission: to educate families, healthcare providers, and communities about Perinatal Mood & Anxiety Disorders (PMADs) and to provide resources to help perinatal people and their families overcome anxiety, depression, and other Perinatal Mood and Anxiety Disorders.

The position is remote and reports directly to the Social Services Manager. This is a 20 hour per week position; limited evening availability is required.

Care Coordination/Professional Resource Management 

  1. Manage the mental health provider directory for PSVa, ensuring that all listed providers are PMAD trained and agree to be listed as a provider. 
  2. Regularly update provider listing to ensure accuracy. 
  3. Communicate with providers regarding PSVa activities and training by working closely with the Communications Manager for inclusion in Newsletters. 
  4. Provide care coordination to perinatal people to reduce barriers to receiving mental health care or other services. This includes: 
  5. Create resource lists by health district for inclusion in the PSVa website 
  6. Assist with referrals for individuals who call the PSVa warm line or attend support groups. 
  7. Collect and review Shelane’s Fund applications. Coordinate paperwork with providers, including ACH payment forms. 
  8. Oversee PSVa’s participation in online referral programs such as UNITE Virginia and Aunt Bertha. 
  1. Ensure all relevant data is collected and reports created for both grantors and PSVa.

The ideal candidate will possess:

  • Thorough attention to detail.
  • Excellent verbal and written communication.
  • Ability to work in a group and on their own.
  • Strong organizational skills.
  • Confidence in decision-making.
  • Familiarity with mental health resources and systems
  • Familiarity with the perinatal time preferred
  • Prior experience with care coordination or case management is preferred.
  • Bilingual (Spanish/English) applicants are strongly encouraged to apply.
